  • Contact details of the requester/person in charge such as phone number and/or email address.
  • A detailed description of the issue and the specific system involved (e.g., Alarm System, Access Control System, Surveillance System, Data System, Audio/Video/POS System).
  • The urgency of your request.
  • Attached files, if any, will also be helpful. Accepted file types include jpg, png, pdf, and doc. Maximum file size 5MB.
